Payment & Appointment Information
Hours
Normal clinic front desk hours are from 9:30 am to 5:30 pm, Monday through Friday.  We can arrange evening appointment times, and we are often open later in the evening to answer calls.  The office maybe closed during lunch hours or for other reasons.  Please call in advance before stopping by the office for a formula refill. 
 
Common Courtesy
Please call two days in advance to cancel or reschedule an appointment.  We have a very small staff, and this helps us a lot!  Appointments fill quickly, and this ensures that those in need can get the attention they seek.  Please call the office if you are going to be arriving late or have difficulties finding the office.  Please understand that we have up to a two month waiting list for open appointments.  Our phone number is 402-933-6444.
 
Business Policies

Payment is due at time of office visit.  Payment must be made in cash, check or credit card.  We take Visa, Master Card and Discover.  A $30.00 service charge will be automatically charged to any returned checks.  Payment must be made in full before the next office visit.  We have payment plans available for all who need special arrangements.  Discounts are available for those under economic hardships, just let us know in advance.

All opened herbs and custom herbal formulas are nonrefundable.  All custom compounded herbal formulas must be paid for in advance of ordering, because we cannot reuse them.  Refunds cannot be made for past appointments.  Not giving a proper 24-hour cancellation notice for appointments or not showing up for an appointment will result in an automatic $35.00 rescheduling fee for the first missed appointment.  Second occurrence of missing an appointment without a 24-hour rescheduling notice will result in full consultation charge.  This charge must be paid before another appointment can be rescheduled.  Our office is very busy and our patients have serious health needs.  No show appointments prevent those who are in great need from getting into appointments.  This policy is a reflection of our dedication to patients and their healing journey.

Refunds for package deals will be charged $135 for the initial appointment and refunded the remaining balance.  With one follow up appointment, the patient will be charged an additional $70 and refunded the remaining balance.

Insurance Reimbursement
Patients often ask if insurance reimbursement is possible.  If a medical doctor will write a prescription for nutrition counseling, there is a possibility your insurance company may reimburse a percentage of your office visit charge.  Please bring the prescription with you! It depends on your insurance company and your plan.  We provide a bill that you can send as a claim to your insurance company.  We do not bill the insurance companies or directly contact insurance companies.  Patients should be aware that nutrition counseling can sometimes be filed as tax deduction under medical expenses (categorized under nutrition counseling).  Appointments qualify to be paid for under Pay Flex and other Health Spending Accounts.
